Payment
Cash and all major credit/debit cards are accepted for payment.
Insurance
Currently accepting Aetna, Cigna, Medicaid, Florida Blue, United Healthcare, and Humana.
Cancellation Policy
Cancellations must be made no less than 24 hours in advance. If you cancel an appointment without notice you will be charged $25.00 and must pay this fee before scheduling a new appointment.
